Online Access

 Patients can now register for a online account to order repeat prescriptions, book Doctors Appointments , inform the practice of any changes to their contact details and access elements of their medical records.  If you wish to do so please read the the information below.

To set up an account you will need to obtain a registration form from Reception at the surgery or download and print from the link below and once completed return it to the surgery.  You will then be issued with access details to set up an account. You will be required to collect these in person and provide some photographic ID. Please see information below regarding under 16's.

Once you have registered for an account with the practice you can set up your access details and order your prescriptions, book appointments and access elements of your medical record through the links on the home page.

OUR PRACTICE GUIDANCE FOR EMIS ACCESS

 Before you begin to use EMIS access we would appreciate it if you could read the following guidance regarding the facilities available over the internet.

Access to Medical Records -

Repeat Prescriptions

EMIS Access gives you the facility to send repeat medication requests to the surgery.  When the surgery has accessed the request it will display as accepted on your EMIS access home page.  Please note that this does not mean that the prescription is ready.  It merely means that the request is being processed.  We still required two working days to process requests.  Please do not send requests for any medication that is not on your repeat list. All prescriptions will automatically be sent to the Chemist in Hampton in Arden village.  Should you not wish this to be the case pleases state in the comments box that you wish to collect the script from the surgery.

 Change of Address

 EMIS Access gives you the facility to notify the surgery of a change of address, telephone or email contact details. This should only be used to inform the practice of change of details within the local area.  If you move out of the practice area you will be required to register with a new doctor in that area.

Booking Routine Doctors Appointments

We would ask that you enter a reason for your appointment in the box provided when booking an appointment as this gives us the opportunity to ensure that it is appropriate for you to see the doctor rather than a nurse.  Please be assured that the all the details entered are secure and cannot be intercepted.  The practice has a strict confidentiality policy.

Missed Appointments

Please let us know if you will be unable to attend an appointment that you have booked online.  Either cancel your appointment online or call us on the telephone.  This will allow us to offer the appointment to another patient. 

We realise that there are valid reasons for not attending, but we will be monitoring such occurrences on a regular basis.  If you miss an appointment more than three times in one year we will remove your facility to use EMIS access, however you would still be able to book appointments by phone.

Nurses Appointments

Due to the nature of nurses’ appointments we are unable to offer them online.

Under 16’s

Patients/Guardians can apply for EMIS access for the any patient under 16 in their care for access to book appointments and repeat prescriptions.  Once a patient turns 16 they can re-apply for a new password or pin to ensure their confidentiality. Please note that patients must be 18  to access elements of their medical record. If previously registered for online access they will be required to re register and show proof of identification to access their medical records.

Inappropriate Use

We will be monitoring the use of this service and we are sure you will find it most useful.  If however we find that any users are abusing the service, we will revoke your access to the service and you will have to liaise with our reception team for services.

Inappropriate use would be considered as: sending inappropriate or abusive messages, booking appointments and not using them 3 or more times in a year, booking appointments for other family members using your name, consistently booking inappropriate appointments with the doctor.
